How do I play with other guitarists?

This is a ridiculous question, but I try and fail every time. I've learned a lot so far and feel like I should be able to already, but I'm really confused as to how I can watch two people play and they "just know" what to do to jam.

-I know I have to be in the same key as them obviously and i've seen lessons here somewhere about how to extract the notes from a bunch of the chords someone is playing and then improvise, but it doesn't help me in actual application. I've never seen anyone with a piece of paper during a jam session writing down all the notes to see which scale to use. How is it that other people just start playing? I can't figure out how to know which key someone else is playing in. Sadly when I make my own parts of songs and stuff I don't even know how to tell what key it's in. When you play something that has more than 3 chords in it. It doesn't seem to have a "home" chord.
